How the months of bone healing at home are followed after surgery in Surat, and what to do if something is wrong.
You stay in Surat for the whole lengthening phase, usually six to ten weeks per stage, with supervised physiotherapy; you fly home once lengthening is complete and the frame is off, and the bone consolidates at home with remote review. Our proposed protocol is a monthly X-ray with a video review during consolidation, sent by WhatsApp or email. We give your local physiotherapist a written protocol, and you receive a written pathway for red flags. Some problems need an in-person examination.
Reviewed by Dr. Aman Khanna, M.S. Orthopaedics, an orthopaedic surgeon. Limb lengthening is bone surgery, not physiotherapy.
For adults aged 18 and over whose growth plates have closed.
In June 2026 surgeons at the Royal National Orthopaedic Hospital (RNOH) in Stanmore, UK, published a case series in the Annals of the Royal College of Surgeons of England about patients who came back to the NHS after limb lengthening abroad (PMID 42229514). It included seven patients referred since 2021, six of them after bilateral femoral cosmetic lengthening. All needed physiotherapy. Two needed major surgery: a quadricepsplasty for severe knee stiffness in one, and removal and renailing of broken femoral nails for poor bone regenerate in the other.
The RNOH news summary of the study, dated 3 June 2026, adds that the patients had a median of 10 hospital contacts each, that only one returned to the original provider for corrective treatment, and that none had had a pre-travel consultation (RNOH, accessed 9 Oct 2026).
Seven patients is a small series. It cannot tell us how often problems occur, and it says nothing about any particular centre. We cite it for one reason: it shows what is at stake when there is no plan for the months after you leave your surgeon. This page sets out our plan in writing, and you can compare it with the plan any other centre gives you. For the wider risk picture, see safety and risks.
Your surgeon sets the flying date from your wound and X-rays, not from a calendar.
This schedule is a proposed protocol. It is built on how bone healing works, because the literature does not fix the intervals.
| Phase | Video review | X-rays | What is decided |
|---|---|---|---|
| In Surat, the whole lengthening phase | In person with the team | As your surgeon orders; included in the package | Rate of lengthening, alignment, joint movement, frame removal at the target length, fitness to fly, handover pack |
| Consolidation, at home | Every month, with the X-ray | Every month | Bone healing, when to start loading, when the nail can come out |
| Any time | Extra review if a red flag or concern appears | Extra films on request | Whether to treat locally or return to Surat |
Lengthening runs at a set rate, commonly about 1 mm a day in four steps. In the canine experiments that set this standard, a rate of 0.5 mm a day often led to premature consolidation and 2.0 mm a day often caused undesirable changes in the lengthening tissues, while 1.0 mm a day gave the best results (PMID 2912628). The rate has to be checked against the bone often, which is why the whole lengthening phase is supervised in person in Surat. At home, the monthly X-ray follows how the new bone consolidates and when load can increase.
The difficulties that matter during and after lengthening include axial deviation, premature or delayed consolidation, muscle contracture, joint luxation, nerve and vascular injury, and pin-site problems (PMID 2403498). In a review of 22 papers on lower-limb lengthening, delayed consolidation was reported in 8.3 percent and joint stiffness in 18.8 percent of external-fixator cases (PMID 39359360); these are published figures, not our own series.
There is no single standard way to judge regenerate bone. A review of assessment methods suggests that several methods used together may be best (PMID 38524725), and a scoping review of one X-ray method, the pixel value ratio, found no standard calculation and unclear thresholds for safe full weight-bearing (PMID 42147764). We found no published trial that fixes the interval between X-rays or video reviews. The interval above is our proposal, and your surgeon may shorten or lengthen it for you.
If the nail is to be taken out, that is a separate, later operation, timed from X-rays once the bone has healed. It means another trip to Surat, and nail removal is not included in the package price; see what is included.
We compare the length against the plan, the width and density of new bone against the previous film, alignment, the position of the nail and fixation screws, and joint position. You receive a written note that says one of: continue as planned, change the loading, change the physiotherapy, take extra imaging, or come back. Weight-bearing and nail removal are decided by the surgeon from serial X-rays and examination, not from a formula (PMID 42147764).
Photographs help but have limits. Grading pin sites from digital images has shown only moderate reliability between raters (PMID 39411545; PMID 38033920), so a photograph supports a review and never replaces an examination.
We brief your local physiotherapist with a written protocol. We do not name partner clinics. The protocol sets out range-of-motion goals for the knee and ankle, the stretching and strengthening suited to the segment lengthened, the weight-bearing allowed at each stage, wound care, and the warning signs that should reach us the same day.
This matters because joints stiffen during lengthening and movement has to be regained afterwards. In 32 patients having femoral lengthening with the Ilizarov method, knee flexion fell rapidly before lengthening began and the loss continued during it, which is why intensive physiotherapy was advised throughout; most patients regained good knee function after the fixator came off (PMID 18564473). In the systematic review of 795 cosmetic patients, ankle equinus deformity was the most common problem and obstacle (PMID 32670567).
Supervised physiotherapy during your Surat stay is included in the package. Home physiotherapy is not in the package price; see what is included. We cannot supervise sessions we do not see. Your physiotherapist's measurements, your video reviews and your X-rays are how we follow progress.
WhatsApp is not an emergency service. If a sign below is severe, go to the nearest emergency department first and message us afterwards.
| Sign | Why it matters | What to do |
|---|---|---|
| Fever of 38 °C or above, or spreading redness, heat or pain around a wound or a former pin site | Pin-site infection is common with external fixation and can compromise the result (PMID 27174086). | Message us the same day with a photograph. Go to an emergency department if you feel unwell or the redness spreads quickly. |
| New or increasing discharge, pus, a smell, or a wound that opens | Early sign of infection; see the pin-site grades below. | Photograph it, message us the same day, and start the wound care on your sheet. |
| Pain, swelling, warmth or tightness in one calf; sudden breathlessness or chest pain | Blood clots are a recognised potential complication of lower-limb lengthening surgery (PMID 34025795). | Go to an emergency department now. Call your local emergency number for breathlessness or chest pain. |
| New numbness or pins and needles | Nerve injury is among the difficulties listed in the Paley classification (PMID 2403498). | Stop any lengthening turns you have been given. Message or call us on WhatsApp the same day with a photograph. If the symptoms persist or get worse, go the same day to a local emergency department or orthopaedic service. Restart only on the surgeon's written advice. |
| Weakness, or a foot that will not lift | Loss of muscle power can mean a nerve is under pressure or injured (PMID 2403498). | Stop any lengthening turns now. Get examined locally today at an emergency department or orthopaedic service, and message or call us. Restart only on the surgeon's written advice. |
| A cold, pale, blue or numb foot; a tense swollen leg; severe pain not eased by your prescribed medicine | May mean a circulation problem; vascular injury is also in the Paley classification (PMID 2403498). | Go to an emergency department now. |
| A sudden crack, new deformity, or being unable to bear weight when you could before | May mean a problem with the bone or the nail. | Stop loading the leg, message us, and go to an emergency department if the pain is severe. |
| Quick loss of knee or ankle movement | Muscle contracture and joint stiffness are a major concern during and after lengthening (PMID 18564473). | Tell your physiotherapist and message us. Do not wait for the next scheduled review. |
Pin-site infection is mainly a risk while the frame is on, which is during your Surat stay; former pin tracts can still become infected afterwards (grade 6 below). It is commonly graded 1 to 6 on the Checketts-Otterburn scale, as tabulated in a review of pin-site infection prevention (PMID 27174086). Because grading from photographs is only moderately reliable (PMID 38033920), send us the photograph and let a clinician grade it. In the literature on external-fixator lengthening, pin-site infection was reported in 52 percent of femoral and 18.8 percent of tibial cases (PMID 39359360).
| Grade | What is seen | Usual response |
|---|---|---|
| 1 | Slight redness, little discharge | Improved local pin care |
| 2 | Redness, discharge, pain, warmth | Improved pin care and oral antibiotics |
| 3 | As grade 2, with no improvement on oral antibiotics | Pins and fixator can be continued |
| 4 | Severe soft-tissue infection involving several pins, with or without pin loosening | The fixator must be discontinued |
| 5 | As grade 4, with bone involvement visible on X-ray | The fixator must be discontinued |
| 6 | Major infection after the fixator has been removed | Curettage of the pin track |
You receive this as a written sheet before you fly. It is summarised here so you can compare it with any other centre's.

The surgical team reviews and replies in writing, including whether treatment can happen near you or whether you should return to Surat.
How revision surgery is arranged and costed is stated in your written quote. Ask to see that wording before you make a reservation payment.
Costs that sit outside the package are listed on what is included. The stages of treatment are described in the limb lengthening process.
Video and X-ray review cannot examine a limb. They cannot feel a tense compartment, test a nerve, or judge a wound the way hands can. Some problems will need to be examined in person, either by an orthopaedic limb reconstruction service near you or by your returning to Surat. Before you commit, work out which hospital near your home could treat a complication, and how you would reach it. If none exists, weigh that.
Candidacy is decided at consultation, and not everyone who asks is suitable. For other questions on travel, stay and timing, see the international patients page, the United Kingdom and Gulf pages, and the FAQ.
Lengthening is supervised in person in Surat. After you fly home, our proposed protocol is a monthly X-ray with a video review while the bone consolidates, sent by WhatsApp or email, with extra reviews if a red flag appears. Your local physiotherapist works from a written protocol we send. A written pathway tells you which signs need an emergency department and which need a message to us.
The proposed schedule is monthly during consolidation, with extra films if a red flag or a concern appears. We found no published trial that fixes this interval, so it is a clinical protocol based on how bone healing works, and your surgeon may change it for you.
A physiotherapist near you does, and those sessions are not in the package price; supervised physiotherapy during your Surat stay is included. We brief your physiotherapist with a written protocol covering range-of-motion goals, weight-bearing at each stage, pin-site care and warning signs. We do not name partner clinics, and we cannot supervise sessions we do not see.
Go at once for chest pain or breathlessness, a cold, pale or blue foot, a tense swollen leg with severe pain, or fever with spreading redness while you feel unwell. Calf pain with swelling in one leg also needs urgent examination for a clot. WhatsApp is not an emergency service, so do not wait for a reply before you go.
Get examined locally if it is urgent, then send us the details, photographs and X-rays by WhatsApp or email. We reply in writing and advise whether treatment can happen near you or whether you should return to Surat. Your written quote states how revision surgery is arranged and costed; ask to see that wording before you make a reservation payment.

Your surgeon sets the date from your wound and X-rays, not from a calendar. Cosmetic cases usually need 12 to 18 months of recovery in total.
No. Video and X-ray review let us follow progress and adjust the plan, but they cannot replace a hands-on examination. Some problems need to be examined in person, either by a local orthopaedic limb reconstruction service or by returning to Surat.
Reviewed 9 Oct 2026. Complication figures are from the published literature named above and are not Living Taller results. The follow-up schedule is a proposed protocol.
Ask how follow-up would work from where you live. A written quote follows an X-ray review, and candidacy is decided at consultation.
